School nursing - Children and young people

Survey for children and young people
We are currently consulting on the Health Visiting (age 0-5) and School Nursing (age 5-19 and 5-25 for Special Educational Needs and Disabilities (SEND)) services in Wokingham Borough, as part of the process to recommission these services.
This means we are reviewing how they are delivered and what they offer, to ensure they continue to meet the needs of children, young people, and families in Wokingham.
Your views are vital in helping us understand what matters most to those who use the services.
By sharing your experiences and suggestions, you can help shape how these services are designed and delivered in the future.
This survey should take around 10 minutes to complete and your responses will remain confidential.
If you have any questions, please email public.health@wokingham.gov.uk
